Required

• Current Certificate of Registration in good standing, or eligible for registration from the College of Nurses of Ontario.
• Current knowledge and skills in Psychiatric / Mental Health Nursing / Mental Health Act.
• A minimum of 2 years working experience within the last 5 years in Psychiatric / Mental Health Nursing.
• Knowledge of the Recovery Model of Care and Trauma Informed Care approach.
• Experience in Group Facilitation.
• Knowledge of CBT (Cognitive Behavioral Therapy) and/or DBT (Dialectic Behavioral Therapy).
• Knowledge and skill in Metabolic Monitoring for chronic conditions and mental illness.

Duties

• Utilizes the Nursing Process in assessing, planning, implementing, evaluating and measuring patient outcomes. This includes triage of high risk patients prior to, or following a psychiatric consultation or discharge from hospital.
• Performs holistic patient health assessments and health maintenance activities including, screening for the presence of acute or chronic illnesses that may be contributing to the overall mental clinical presentation.
• Performs acts requiring specialized knowledge, skills and judgment in assessing psychiatric/mental health needs and in planning, administering and evaluating patient care.
• Assessment, administration, monitoring/evaluation of medication effectiveness for those undergoing medication adjustment and symptom stabilization.
• Establishes therapeutic relationships as per the College of Nurses Standards.
• Provides appropriate health teaching to patients and families with a focus on medications, acute and chronic disease/symptom management.
• Development of patient focused documented care plans.
• Maintains nursing records.
• Participates in Continuous Improvement programs to ensure a high level of patient care.
• Passion for motivational interviewing and knowledge of the stages of change is an asset.
