Position Summary

The Patient Care Lead is a unit based clinical leader, who embodies the values of Quinte Health Care in ensuring that patient care delivery meets the mission and vision of the organization. The Patient Care Lead is accountable to the Patient Care Manager for the day to day provision of quality patient and family centered care and monitoring clinical outcomes through appropriate delivery of patient care, and development and implementation of unit based quality and process improvements through an interprofessional practice model They will support mentorship for clinical staff in adherence to clinical standards and policies and facilitate an inter-professional practice environment through coaching and “just in time” learning. They will be a resource to patients, families and staff, and facilitate staff learning about quality and performance improvement.

Specifically the Patient Care Lead will:
• Participate in the effective and efficient functioning and evaluation of the unit in collaboration with the inter-professional care team and Community partners,
• Co-ordinate patient care through collaboration with members of the interprofessional care team and act as a resource in developing, implementing and evaluating outcomes of clinical protocols, pathways and pertinent quality patient care indicators,
• Promote optimal allocation of resources both human resources and materials,
• Act as a resource to the inter-professional care team in the provision of safe and therapeutic patient care,
• Work with the inter-professional care team to ensure standards of care are met through role modeling and consultation, including mentorship when required for staff development
• Develop, implement and evaluate quality care initiatives and learning as appropriate,
• Promote and maintain collaborative team relationships,
• Promote best practices for Mental Health through collaboration with the unit and the interprofessional team,
• Knowledgeable of legislation relevant to the Mental Health (eg Mental Health Act), and
• Adheres to supervisor responsibilities as set out in the Occupational Health &Safety Act and hospital safety policies.

Required

• Active registration in the College of Nurses of Ontario as a Registered Nurse.
• Completion or commitment to complete Certified in Psychiatric and Mental Health Nursing Canada through the CNA certification process within 1 year.
• Five years Mental Health inpatient experience in the last 7 years.
• Certified in CPR, NCI.
• Excellent clinical skills.
• Demonstrated ability in critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment that requires problem solving, organization and prioritization.
• Demonstrated excellent leadership skills including the demonstrated ability to role model and mentor.
• Demonstrated initiative to participate in unit activities.
• Excellent communication and interpersonal skill.
• Demonstrated commitment to continuing professional development and education of self.
• Demonstrated team approach and professional demeanor.
• Must have previous proven good performance and attendance record.
• Demonstrated basic computer skills and willingness to learn.
• Completion of CNA certification in Mental Health or registered to complete.
• Completion of relevant Mental Health Courses or registered to complete.

Duties

Clinical Responsibilities:
• Ensures patient assignments are based on the inter-professional care team model, alignment of patient needs and scope of practice/skills of staff, including opportunities for safe learning of novice practitioners.
• In collaboration with the team, adjusts staffing, assignments throughout the shift as required, and anticipates adjustments for future shifts.
• Acts as a clinical resource to staff as required.
• Facilitates effective communication among interprofessional care team members.
• Reports problems affecting patient care of the unit to the manager.
• Ensures patient care plans are current and accessible to patients, families and the members of the interprofessional care team.
• Encourages staff to participate in rounds, huddles and sharing information with other team members.
• Ensures discharge planning is interprofessional and conducted in an efficient and effective manner using hospital and community resources as necessary.
• Collaborates with Patient Care Leaders on other units/areas in regards to shared clinical issues.
• Supports and participates in organizational resource allocation activities identifies and participates in the implementation of solutions for clinical utilization and process improvement.
• Participates in the development, implementation and evaluation of relevant policies, procedures, guidelines and standards of care using best practice evidence.

Educational Responsibilities:
• Maintains professional self-development through attendance at in-services, seminars and courses.
• Shares information obtained at educational sessions with colleagues.
• Assists manager/clinical educator in identification of any areas of clinical practice that need development.
• Shares clinical knowledge and expertise with staff through incidental teaching.
• collaborates with the interprofessional care team members to ensure patient teaching programs are implemented.
• Participates in nursing education program by collaborating with Manager/Clinical Educators, and instructors to help identify preceptors and appropriate patient assignments for students.
• Facilitates orientation of new employees/students.
• In conjunction with Manager/Clinical Educator, co-ordinates staff attendance at in-services.
• Facilitates staff to attend/complete mandatory in-services.

Quality and Safety Responsibilities:
• Ensures accurate and complete communication and follow up of any issues which may have arisen from previous day i.e. patient/family concerns, incidents, change in patient acuity, etc.
• Participates in the review, investigation, follow-up and documentation of all incident reports, including staff injury reports.
• Ensures staff provide factual, timely information and documentation regarding patient and staff incidents.
• Participates in quality and process improvement initiatives-facilitates excellence in clinical utilization by assessing, monitoring and evaluating clinical processes.
• Actively participates in ensuring a constant state of improvement by compiling and analyzing monthly metrics to achieve identified unit goals, resulting in enhanced outcomes, quality patient care and patient safety.
• Promotes a safe environment by informing manager of any relevant issues related to patients, families or staff.
• Participates in weekly and monthly unit environment and equipment inspections and oversees and assigns unit audits including but not limited to hand hygiene, patient satisfaction surveys, duress device management.
• Notifies the appropriate departments of equipment on the unit that requires repair/maintenance.
• Ensures safety issues are dealt with promptly.
• Other duties as assigned by the manager including leading and role modeling unit huddles.
