Position Summary

As part of the Quinte Health team, the Clinical Projects and Operations Coordinator will report to the North Hastings Hospital (NHH) Manager to lay out and implement plans for growth, and innovation that will keep pace with the community and region. As an integral team member, you will reflect our strong culture and values that we bring to each interaction with patients, each other, and with our partners across the health system.
The NHH Clinical Projects and Operations Coordinator will assist the Manager to ensure resources are in place to support timely, and appropriate patient care, take an active role in the coordination of professional growth and development of unit staff to promote continual improvement in clinical practice and quality patient care.
The Clinical Projects and Operations Coordinator in collaboration with the Hospital Lead Patient Services Manager will take a leadership role in change management and, planning and implementing priority projects for the hospital including Lumeo and GotCare Virtual Remote Care Monitoring while supporting other clinical priorities.
Gotcare’s digital health solutions project coordinator will lead North Hastings in streamlining the process for providing care for patients with chronic conditions who reside in rural areas for safe diversion of admissions and between discharge and implementation of home care working with the project team implementing the Digital Supercluster funded project.
For Lumeo this project coordinator will take a leadership role supporting the implementation and change management of Lumeo at North Hastings site.

Required

• A Baccalaureate Degree is required, BScN preferred.
• Demonstrated effective communication, team building, problem-solving, and conflict resolution skills.
• Demonstrated ability to engage and work collaboratively with all levels and members of the interprofessional team.
• Demonstrated effective judgment and innovative change leadership skills.
• Demonstrated ability to utilize quality, safety, and risk-based frameworks to facilitate and build the use of critical thinking, priority setting, and problem-solving skills across the team. Knowledge of collective agreements and working experience in a unionized environment is preferred.
• Excellent written, verbal communication, and documentation skills. Detail-oriented with excellent organization and time management skills.

Duties

Quality and Safe Patient Care
• Support the implementation of best/leading practices, quality documentation, and accountability models;
• Under the guidance of the manager, monitor and respond promptly to quality, safety, risk, and other performance/outcome indicators such as hand hygiene, incident review
• Promote patient and family-centered care.
• Respond to and address issues and/or concerns identified by patients and families and internal and external stakeholders in a timely fashion;
• Coordinate resolution of issues/concerns/ related to operational activities
• Work collaboratively with the inter-professional team members
• Other duties as assigned such as special projects.
• Liaise with external community organizations in a common goal to support patient care
Leadership and Collaboration
• Provides project management leadership of the Lumeo (HIS (Hospital Information System)) and GotCare Virtual Care implementation for North Hastings Hospital
• Leads and works collaboratively with the project teams and stakeholders to provide expertise and coordination of all aspects of Lumeo, GotCare Virtual Care, and other clinical priorities for North Hastings Hospital
• Demonstrate ability and experience in five leadership domains: Leads self, engages others, achieves results, Develops partnerships, and leverages information-driven and goal-oriented processes in driving change and improvement
• Works collaboratively with the manager toward achieving program targets, managing risk, improving quality and safety, and advancing patient care
• Works collaboratively with the Patient Care Leads, Clinical Resource Nurse, and Professional Practice Specialist in reducing waste, adding value, raising standards, improving effectiveness, and delivering high performance from the team;
• Works collaboratively with the manager and program Director to identify gaps in processes, policies, and procedures and support the sustainable delivery of excellent patient-centered care;
• Facilitates huddles and chairs the nursing team meetings under the direction of the manager and attends other internal/external meetings as appropriate
• Participates in and coordinates group meetings, including tracking and minuting action plans.
• Works collaboratively with Quinte Health partners to arrange and facilitate health and safety needs, and requirements for safe work practices
• Maintains oversight of equipment utilization and supports the Manager in capital equipment planning
People Development and Team Building
• Fosters open communication, learning, and professional growth of the staff.
• Supports effective change management within the multidisciplinary team
• In collaboration with the manager, supports the recruitment and selection process
• Provide support to the manager in completing performance evaluations.
• Cultivates effective interprofessional relationships at all levels.
• Creates an environment that empowers individuals to take ownership and responsibility for their actions, promotes staff initiative, and nurtures high morale and engagement.
• Demonstrates relevant ongoing professional development and commitment to continuing development
• Responsible for supporting the Manager with the completion of timekeeping, shift changes, payroll completion, and approval
• Collaborates with Professional Practice and the multidisciplinary team to help arrange and organize events both educational and organizational initiatives

North Hastings Hospital (NHH) is located in the small, picturesque town of Bancroft. It sits on the edge of Algonquin Park, one of the largest provincial parks in Ontario, featuring numerous lakes, rivers and a range of wildlife. Situated in a beautiful part of Ontario's 'cottage country', Bancroft boasts a relaxing lifestyle, a close-knit community, and provides opportunity to explore nature, snowmobile on incredible trails, and participate in fishing and water sports on various lakes.North Hastings Hospital is the smallest and most rural of Quinte Health's four hospitals, and works closely with the Bancroft Family Health Team. It is a six-bed primary care hospital that operates a busy, 24/7 emergency department (including emergency obstetrics). The inpatient beds are available for medical and palliative care services. North Hastings Hospital also provides diagnostic services, physiotherapy, outreach clinics and telemedicine services. NHH works closely with the other three Quinte Health hospitals, as well as regional health centres, to serve the community together.The North Hastings Hospital team is truly like one big family. They not only work together, but often spend time together within the community.
