Position Summary

As a Wound Care Nurse, you'll be at the forefront of innovative wound assessment, treatment, and healing techniques. Beyond bedside care, you'll lead in disseminating best practice knowledge, educating teams, and collaborating on care plans.

As an advanced practice clinician, you bring a unique blend of clinical expertise, compassion, and advocacy to every interaction. With a background as a Registered Nurse and specialized training through the Nurse Certified in Wound, Ostomy, and Continence (NSWOC) program, you're equipped to make a real difference in the lives of patients with complex wound care needs. The wound care nurse's role is to understand and share best practices and educate the teams on them both via the creation of policies and education resources and also through in-person, hands-on training with teams. Our wound care nurses are leaders in the interdisciplinary care team regarding wound care and are expected to guide the physicians, nurses, etc. through the care of patients with complex wounds.

Required

• Bachelor of Science Degree in Nursing.
• Current Certificate of Registration from the College of Nurses of Ontario.
• Current CPR or BCLS certificate.
• Minimum of 2 years current and related clinical experience.
• Nurses Specialized in Wound, Ostomy and Continence Canada (NSWOCC) certification.
• Demonstrated skills in patient and staff education.
• Participation in local and/or national NSWOCC and wound care associations.
• Evidence of continuing professional development.
• Demonstrated leadership skills.
• Demonstrated ability for independent action, initiative, problem solving, organization and prioritization.

Duties

• Uses appropriate assessment tools and techniques in consultation with patients and the health care team to assess patients regarding physical, emotional, spiritual, cultural, cognitive, developmental, environmental, and social needs.
• Maintains the knowledge, skills, and attitude to provide safe, efficient, effective care in all areas of practice by keeping abreast of changes in the practice setting that affect scope and practice.
• Develop quality and performance tools as it relates to wound, ostomy, and continence care.
• Provide leadership, mentorship, education, and support to the nursing, medical, and allied staff.
• Provides expert clinical consultation (assessment, collaboration, education, interventions) and care planning for complex wounds, ostomies, and continence.
• Lead evidence-based practice for clinical teams and the development of standards and quality initiatives enhancing safe high quality patient and caregiver centered care.
• Provides comprehensive assessments to identify symptoms, underlying health conditions, physical, functional, emotional and cognitive status, home environment issues and home supports needed for patients with complex wounds and ostomies.
• Compile and assist with the reporting of wound, ostomy, continence metrics, participate in/conduct research and will participate in product selection for wound/ostomy products.
• Will work with community providers to ensure the continuum of care for patients.
• Works with Managers, Professional Practice staff, clinical staff and members of the interprofessional team to assess, plan, develop and evaluate learning strategies to meet their learning needs in delivering quality and safe patient care.
• Incorporates learning principles, organizational development, behavioral and physical sciences, communication and change management in planning, designing, delivering and evaluating educational programs using multiple methodologies.
• Identifies and works collaboratively with the interprofessional team to develop learning resources as required to support clinical practice.
• Provides education/counseling and support for patients and families.
• Participates on committees as appropriate.
• Participates in research and continuous improvement programs/initiatives.
• Uses the Nursing Process for the development, implementation and evaluation of patient care plans and ensures clear communication within the health record.
• Engages in risk assessment protocols when determining and addressing patient care needs.
• Acts as a patient advocate and liaises with community care providers as appropriate.
• Participates and/or leads multidisciplinary patient-centred case conferences as required.
• Other duties as assigned.
