Position Summary

The primary role will be to support the post-acute rehabilitation program for total arthroplasty patients (hips,knees and shoulders) as part of Quinte Health’s bundled care program. The physiotherapist will implement post-acute rehabilitation programs in the outpatient setting at Quinte Health. Ongoing care and assessment in addition to completion of the mandatory discharge assessment and evaluation criteria will be required in accordance to QHC’s accountabilities to the MOHLTC.

In addition to the duties of the outpatient physiotherapist, the incumbent will provide support to the Quinte Health central intake and assessment clinical as an Advanced Practice Physiotherapist (AAP).The Advanced Practice Physiotherapist will apply advanced knowledge and decision-making skills for assessment, therapeutics and management of the patient with pre-operative musculoskeletal and orthopedic referrals for primary arthroplasty specific conditions.

It is expected that this individual will participate in a mentorship program with the orthopedic surgeons to acquire advanced competencies that will allow the ordering and interpretation of standardized diagnostic imaging and laboratory investigations and to identify the indicators that a patient would likely benefit from a surgical intervention. The Advanced Practice Physiotherapist will collaborate with healthcare providers including family physicians, orthopedic surgeons, physiotherapists, occupational therapists, nurses,
along with patients and their family members. In addition to these responsibilities the Advanced Practice Physiotherapist will mentor colleagues as required.

Required

• Current Registration in good standing with the College of Physiotherapists of Ontario.
• Advanced Practice Physiotherapist designation, or willing to obtain
• Minimum of 5 years’ recent musculoskeletal clinical practice experience and evidence of participation in clinically relevant continuing education activities.

Required
• Current Registration in good standing with the College of Physiotherapists of Ontario.
• Advanced Practice Physiotherapist designation, or willing to obtain
• Minimum of 5 years’ recent musculoskeletal clinical practice experience and evidence of participation in clinically relevant continuing education activities.
• Current membership in the Canadian Physiotherapy Association (CPA) with evidence of current malpractice liability insurance.
• Respects the value and contributions of the interdisciplinary healthcare team.
• Demonstrates ability to deal effectively with change.
• Experience in developing and implementing strategies to improve access to care.
• BCLS at the Health Care Provider level.
• Proven good attendance
• Excellent verbal and written communication skills.
• Ability to organize, prioritize and meet deadlines.

Duties

• Provides services in a manner that is consistent with Quinte Health’s Mission, Vision, and Values and reflects the core values of Patient-and Family-Centered Care (dignity and respect, information sharing, participation, and collaboration).
• Possess and demonstrate: advanced knowledge, skills, and clinical reasoning specific to musculoskeletal conditions, grounded knowledge of theoretical foundations, experience, and research, be responsible for advancing clinical excellence, and best practice and innovative care models.
• The Advanced Practice Physiotherapist will assess and triage patients referred to orthopedic surgeons and provide education, conservative management advice and consultation prioritization to see an orthopedic surgeon. Those patients not deemed ready to consider surgery will be recommended for routine symptom monitoring through their referring Health Care Practitioner.
• The Advanced Practice Physiotherapist is encouraged to participate in relevant educational opportunities and demonstrate a strong desire and commitment for self-learning.
• The Physiotherapist will implement the outpatient rehabilitation pathway for patients referred to the outpatient program.
• Will use standardized assessment tools and patient care pathways adopted from the Rehabilitative Care Alliance specific to each disease site in both the CIAC APPT role and in the outpatient surgical Physiotherapist role.
• Co-ordinates and monitors patient access to care in wait 1 and 2 by using established performance targets.
• Maintains all records and data reporting elements as determined by the Central Intake program and will use regional standard forms for documentation/referrals etc.
• Functioning within an interdisciplinary team, the Advanced Practice Physiotherapist will use specialized expertise to conduct standardized physical and functional biomechanical assessments. The process of assessment will include the review of diagnostic imaging and other clinical reports.
• Liaise with and collaborate with the primary care provider who submits the CIAC referral and will provide recommendations for follow-up for non-surgical arthritic candidates and the use of self-management protocols. Duties will include knowledge of and recommendations for community-based programs that would support non-surgical patients.
• Assists the Program Manager in the performance and quality improvement responsibilities and participates in regional program planning, change management and evaluation activities associated with the CIAC.
• Participates in program and hospital-wide initiatives that model and promote a safe and healthy patient-centered environment that contributes to quality improvement.
• Wherever possible, employees will ensure patients and families receive complete and accurate information and have the opportunity to ask questions, encourage patients and families to participate in care and decision-making at the level they choose, and collaborate by involving the patient and family in the formulation of the care plan.
• Assists Program Manager with policies and procedures.
• Evaluates new procedures and equipment and insures that equipment is maintained in good working order.
• Responsible for ensuring all the safety guidelines/requirements are adhered to.
• Performs other duties as assigned from time to time.
• Follows safe working conditions and practices in respective areas of responsibility.
